Our five-year plan. Better quality of life.
PreventionLink of Southern Maryland has a five-year plan to identify and improve the quality of life of residents living in underserved communities who are either at risk of or living with a chronic condition by providing easier access and affordable solutions to good healthcare.
The main goals of PreventionLink:
- Preventing or delaying type 2 diabetes among at-risk individuals
- Helping participants get their A1C (blood sugar) levels to a healthy range to reduce their risk of heart disease and other complications such as nerve damage and vision problems
